Michael O’Neill reports from the Hillsborough stadium disaster (15/04/89) inquiry in Sheffield on the 3 days of evidence by Chief Superintendent David Duckenfield who was in charge of operations on the day. He had to admit it was a mistake to not get stewards to send fans around the side, apologise to Liverpool fans for blaming them and admitted to lying about the opening of the outside gate.
