Femoro-Tibial Bypass Using Human Umbilical Vein

Synopsis
Limb salvage for severe ischemia frequently requires a femoro-distal bypass. Sometimes the most suitable vessel is a distal-tibial vessel and magnification techniques may be needed for anastomosis. The autologous vein is the surgeon’s preference for a femoro-distal bypass in this case. The autologous vein can be used in situ but other surgeons may prefer to reverse the vein. On this occasion, the patient requiring limb salvage has had a venous thrombosis in the past and his long saphenous vein was thrombosed. An alternative was therefore required and human umbilical vein was selected. The approach to the femoral vessels in the upper anastomosis and the approach to the distal posterior tibial artery is demonstrated. Details are shown of the precise anastomosis technique with special reference to the use of umbilical vein as prosthesis.
