Synopsis
Investigates patterns of human error and the reasons why things go wrong. Considers whether some people are more accident-prone than others and whether mistakes can be prevented. Showing a housewife, a piano student and a conjuror, revals how our carefully learned routines can lead us astray. Uses a detailed reconstruction to demonstrate how such a series of lapses cause the air crash in Tenerife in 1977.
