- Synopsis
- Looks at engineering failures and asks if anything can be done to prevent or reduce them. The collapsed hotel walkways in the Hyatt Regency Hotel, Kansas City, Missouri, in July 1981; the failure of an American telephone system and the crash of the computerised ambulance despatch system in London, are all engineering diasters that occurred because of poor design, specification and implementation.
