Description
A report on the inquiry into the death of 8 year-old Lester Chapman, whose body was found in a ditch. A 68-page report was compiled on the troubled Chapman family as there were suspicions that social workers should have acted more promptly. The inquiry team was led by London barrister John Hall QC and included a magistrate from Oxford, an area nurse, a consultant paediatrician, and a social work officer. It was found that there were no reckless or negligent breaches of duty by anyone. However, both Hampshire and Berkshire County Councils and Area Health Authorities say there were instances noted in the report in which different decisions could have been reached.
