Paul Woodley reports as a series of mistakes led to US friendly fire on two American helicopters flying over Iraq. The helicopters were mistaken for Iraqi aircraft defying the UN no-fly ban in the region. 26 people including 2 Britons died. Chairman of the US Joint Chiefs of Staff, General John Shalikashvili; US Defence Secretary William Perry; and British Defence Secretary Malcolm Rifkind comment.
