Jean Ritchie QC led the independent inquiry into the care and treatment of Christopher Clunis, a schizophrenic who murdered Jonathan Zito at Finsbury Park tube station in 1992. She speaks about the recommendations the inquiry has made such as a national register for the mentally ill and she highlights the catalogue of errors which allowed Clunis to be released into the community.
