A 21 year-old mental patient was mistakenly released on home leave from hospital in South London. He went on to kill two people and injure three others during a week of violence a year ago. Wayne Hutchinson was today remanded to Broadmoor pending sentencing on two counts of manslaughter. An inquiry has now been launched into why Hutchinson was allowed to take home leave after absconding from hospital on several occasions.
