Synopsis: This highly interactive DVD takes a real incident-a ladder accident involving a maintenance worker- and fictionalises all the events surrounding it to show the complete catalogue of errors that occur in the minutes, hours, days and weeks prior to the accident. As in a real investigation, what trainees discover is entirely reliant on what they choose to explore. By deciding who and what might be significant and then clicking on the screen to investigate, trainees use their initiative to establish the immediate and underlying causes of the accident. The DVD package includes a second disc with solutions to the issues and four guides for tutors and trainees: ASI-Timeline, ASI-Note Form, Accident Investigation Checklist and ASI-Guide. The latter explains how the on-screen hot-spots can be clicked to reveal clues, how to get trainees into the investigative mindset and highlights from users on how they have used the film for their own training sessions.
