Synopsis
A collection of ten health and safety films. 1. TAKING LIBERTIES: A FATAL ERROR. In 1989 a British Midland plane crashed into the embankment of the M1 near Kegworth in Leicestershire with the loss of 47 lives. The investigation which followed concluded that pilot error was the primary cause, but was it? 2. A CUT PRICE TRAGEDY: looks at the tragedy of Valujet DC-9 that opened the case of safety and responsibility in the air. 3. KING’S CROSS FIRE: BENEATH THE FLAMES: A look at what happened in 1987 when fire broke out at King’s Cross, one of London’s busiest Underground stations. 4. A CANCER IN THE SYSTEM: investigates when women died because their cervical smear slides were confused and incorrectly analysed. 5. SPIRAL TO DISASTER: recreates the disaster of the oil rig ‘Piper Alpha’ and is used by health and safety professionals all over the globe. 6. The CHANNEL TUNNEL FIRE: reconstructs the fire in 1996 on the Eurotunnel shuttle where 34 people were trapped in the carriages. 7. A MAJOR MALFUNCTION: looks at the drama leading up to the final countdown of the doomed launch of the 25th US Space Shuttle ‘Challenger’. 8. FIGHTING FOR LIFE: two surgeons at Bristol Royal Infirmary were heavily criticised for operating on babies despite a poor survival rate and a third for failing to act on concerns expressed to him. 9. KANSAS CITY: DEATH BY DESIGN: portrays the catalogue of horrific human error, architectural flaws and tragic occurrences that led to the deaths of over 100 people as two of the walkways in the Hyatt Regency hotel’s atrium collapsed. 10. DEEPWATER DISASTER: engineers and oil men at the heart of the operation talk for the first time about the colossal engineering challenges they faced and how they had to improvise under extreme pressure.
