Malignant Melanoma: Cosmetic Wide Excision, Complete Lymph Node Dissection

Synopsis
Presents a personal view of two aspects of surgical management of malignant melanoma. The operative procedure is shown after removal of the primary lesion. A deep, wide excision is made around the primary site and the edges of the wound ‘saucerised’ by suturing to provide a shelved edge to the subcutaneous tissue. This assists healing and filling of the crater. A skin graft is placed over the site and attached by means of a kind of superglue. Follow-up pictures taken after several years are shown to demonstrate good wound healing. X-ray films taken during lymph node dissection demonstrate complete clearance in axilla or groin as appropriate. Several cases are shown and clinical problems discussed.
